He was born in Middleburg Heights, Ohio on September 29, 1975.
Michael married Dawn Salsbury in 2008 and was a US Navy Veteran.
He is survived by his beloved wife Dawn; loving daughters Summer Lane Stephenson and Shelby Alyssa Sherman; brother Kip Slapakovs, mother Candice (Daniel) Dellanno; grandparents Gerald and Darlene Ashie; and was a dear and loving relative to many.
Private family service will be held at a later date. Arrangements have been entrusted to Carlson Funeral Homes & Cremation Services, 1642 Pearl Road (½ mile South of Route 303), Brunswick.
